Dysport Redemption Program.

Dysport is offering a coupon program which works by providing our patients with a rebate check limited to $50.00 for one treatment with Dysport.  This offer is limited to patients over the age of 18 who are treated with Dysport.

To participate in this offer our patients must receive a Dysport treatment between May 1, 2012 and June 30, 2012.  The Rebate Redemption Form is available at the time of the procedure.
